For your convenience, we have enclosed a Request for Release of Medical Records form. If you would like to request a copy of your medical records or request that we forward a copy to another physician, please fill out the enclosed form and send it to us via e-mail at idgrcm@derm-services.com, or by mail to Integrated Dermatology of Boise, PLLC c/o Records Administration PO BOX 947977, Atlanta, GA 30394-7977.
